She was admitted to the hospital on April 4th, on April 6th she tested positive for COVID-19. On Monday and Tuesday of that week Patsy FaceTime’d with her family and seemed to be recovering but her situation declined and by Friday she was in ICU on a ventilator. Patsy rallied and was able to come off the ventilator a week later but too much damage had been done and she passed away about 2 days later. Patsy was the mother to 3 sons, she was Mamaw to grandchildren and great-grandchildren. She has a sister and brothers along with many friends that will miss her and we will mourn their loss as well. Let’s make sure we are thinking about Patsy. When we lose someone, it’s not just a number but an individual that will be missed.

  • No. 1 Healthy at home. Number one directive. We need to be healthy at home. This is how we protect each other as an American, a neighbor, this is how we save lives. You staying home is a heroic gesture. 99.99% of Kentuckians are doing your duty, and thank you.
  • Limiting those contacts is absolutely critical. If you’re not healthy at work, exercising, or getting essential supplies, be healthy at home.
  • No. 2 Avoid crowds and gatherings. Again, no exceptions and you all have been phenomenal about it. There isn’t a number that is OK. All crowds, all gatherings, don’t be a part of it. It protects you and the people around you.
  • No. 3 Social Distancing, don't make this an excuse to go to places where people are congregating. We have been able and committed to doing the right thing. Know when to seek care. We have some people that are coming to the ER really late. Seek care if you really need it.
  • No. 4 Know when to seek care. - If you are healthy but nervous then call the coronavirus hotline (800-722-5725), if you are sick but wouldn’t have otherwise sought care then call your primary care physician, but if you are truly sick or injured go to a healthcare facility.

  • “Healthy At Work”

  • We have to do it smarty, safe, and gradual.
  • None of us want to reopen and set us back, a spike with more people that die.
  • Tomorrow, the Chief of Staff will talk about some of the discussions we have had.
  • Our goal is to have industry guidelines (mandatory ones), as we can open up different areas and professions.
  • It will be gradual, phased, not everyone will be able to open up in the very short term.
  • Doing it right is about saving lives and that employees will be safe and the people they serve will be safe.
  • Today: We will talk about the first phase. First reopening in our Healthcare area.
  • This is our “proof of concept” If we create the right guidelines and enforce those, and do it gradually, we can do it safely.
  • This is no mistake that we are doing it in the healthcare area. This is the fastest to respond to any issues, the most aware of PPE and procedures, and will give us a good look as to what this can look like as we do it in other areas.
  • If you are a practitioner in these areas, make sure you’re fully committed to following these guidelines.
  • We had to turn our guidelines into a mandate because not everyone was following them. Don’t be that person. Don’t be the folks that could ruin it for an entire profession and set back our timetable. These guidelines will be on the website.
  • Dr Stack will walk through who is included and what is expected.
  • I can’t go into the level of detail in 5 minutes, but I will attempt to do the most. Slides will be online.
  • Please look for the guidance which will be available ASAP.
  • You’ll remember the difficulty we had in shutting everything down. More questions are coming than we can give answers to, thank you for working with us with patience. It’s difficult to turn things off and then back on again.

  • Resuming: non-urgent health care, for certain settings. On Monday April 27

  • Physical therapy settings, chiropractic offices
  • Optometrists
  • Dental offices (but with enhanced aerosol protections)
  • Separate guidance for (list of facilities/types) LTC, Prisons, etc.

  • Phased gradual reopening.
  • In all cases, maximize telehealth rather than in-person services.
  • No visitors except for end of life, minors, etc, and only minimally as needed
  • Eliminate waiting areas or common seating areas. Use non-traditional alternatives (e.g. parking lot lobby)
  • Maintain social distancing in all settings where people are just waiting and minimize direct contact between individuals within the healthcare setting.

  • Screen all workers, patients, and others for temp.
  • Require staff to stay home if sick
  • Plan for enhanced sanitizing in the workplace
  • Plan for enhanced hand hygiene (washing & sanitizing)
  • Provide sanitizer through the office/building
  • Procure PPE via normal supply chains
  • The state will not be able to buy this all for you and supply it, you have to be able to provide your own PPE for operations. The state’s supply is not meant for general use.

  • Masks; will be more a part of your life. (We are not wearing masks right now so that you can see and hear us more clearly.)
  • All providers must wear surgical/procedural masks.
  • All patients or others in facilities must wear surgical/procedural masks in ambulatory surgical centers.
  • In other healthcare settings, wear at least a cloth mask.
  • We are attempting to do source control; minimizing what comes from you giving to someone else.

  • Special Considerations:
  • I don’t want to call out single sites of service or industries; but there are times when there are unique distinctions and they have to be identified.
  • “High Touch Settings” If you are in intense contact with someone, you have to take extra precaution. Wear gloves, sanitize equipment
  • “High-aerosol risk outpatient settings (Detisty, oral surgery, anesthesia, pulmonary services, etc)
  • We are looking forward to everyone's input. Dental offices are more intense because you are constantly spraying oral secretions. There will need to be additional protections, such as N95 masks on staff directly providing services. Airflow is something to be addressed as well.
  • We are not trying to single out dentists in a special way, but you have additional concerns and challenges. Because this is qualified, you would be incorrect to assume that you can open Monday with normal procedures.
  • We can’t shout out to everyone, but thanks to (Kentucky Medical Association, Kentucky Hospital Association, Kentucky Association of Chiropractors, Kentucky Chiropractic Society, Kentucky Optometric Association) are some of the input received.
  • This stuff is read. We rely on you and we work with you, but we need to work together.
  • Dr Stack made a good point. Your business cannot look the same when you are able to gradually restart than before.
  • Waiting rooms is where it spreads. All the people that used to come in; NO. If they can telework, do so. If you are envisioning reopening the same way, it will not work.
  • Make sure you are going above and beyond. This is our proof of concept. In the medical community where they are used to dealing with contagious viruses, we have to prove that we can do this and we can do it in the right way.
  • Make sure you’re committed and you’re thinking about every part of the business. These are requirements. Our job is to make sure people are as safe as they can be.
  • People are getting sick with other things and we need to make sure we are taking care of them as well.
  • I want to thank Kentucky Organ Donor Affiliates and Kentucky Circuit Clerks for donating 1,500 COVID-19 swab test kits.
  • Transplantations happen even in the midst of this pandemic.
  • Although the Circuit Clerks are not currently issuing driver's licenses they have helped addo ver 2 million KY added to the donor registry.
  • Every year, April is National Donate Life month.
  • Before we get to the numbers: what we know is that this virus is hitting our LTC really hard. Not just us, everywhere. We saw it happen in Washington first, and it’s hard. Difficult logistically, emotionally, hard on the individuals and families.
  • I’m going to have Sec. Friedlander came to talk about some specifics of some facilities.
  • Without the work they are doing we would be losing more people. The virus comes for our most vulnerable people. Our job is to fight as hard as we can to protect as many as we can.
  • First, I want to say that we are working very hard to address the issues seen in LTC. We would not be in as good of a shape as we are today without stopping visitation as early as we did.
  • This is NOT an area we are talking about opening. We are being more careful.
  • More testing; entire facilities.
  • We are beginning and working on ramping up testing for ALL LTC, and a tiering system for testing those that need the most help. Any time a facility has a positive resident, we work with the facility to figure out their needs and how we can support them.
  • Norton Healthcare has provided support to making decisions on providing care, when to transport, when to ask for more testing, and when they need more support.
  • We’ve found that on many calls, that support is most needed. We’ve made changes to what we allow in medications. We’ve been working with how we work on our Strike Teams that will sometimes visit virtually, sometimes directly. We’ve provided staffing support to several facilities.
  • Task Force guidance is out. It’s on the inspector’s generals website.
  • We will provide additional behavioral health support.
  • Lastly, Jackson Manor; I reached out to Judge Gabbard. Had a good conversation. At the end, he was letting me know how much they appreciate their support. Jackson Manor will have many people retested. We've provided some PE to Jackson Manor.
  • All LTC facilities need to report through our web EOC which every nursing facility needs to do every day. We need to know what your rate of use of PPE is so we can appropriately provide support for PPE if they need. They need to report on a daily basis. Communication is important.
  • “WEB EOC” is our portal for PPE reporting/burn rate. That’s how people report to us so we send it to the right place.
  • Today’s numbers are better news than yesterday. It’s hard to report as “better” even when there are positive cases, but are lower.
  • We are announcing 161 positive cases today
  • Removing duplicates, our total is: 3,481 cases.
  • 42,844 confirmed tested so far (Increase of 6,769)
  • In the last few days, we’ve had a good increase in our testing.

  • Green River Correctional: 1 new case (1 new inmate). 50 total, 32 inmate, 18 staff, 1 deaths.

  • Long Term Care Facilities: 32 new residents and 27 new staff positive from yesterday, and 9 more deaths. 530 residents, 251 staff, 86 deaths
  • That death number is higher than today because it comes a few days later from LTC.
  • 44%-ish of our overall deaths are LTC. This is where the virus truly devastates.
  • While we reopen, someone could be asymptomatic and spread to a LTC facility. This is why we have to be careful.
